Postmortem-to-Runbook Generator Prompt

Turn the diagnosis and mitigation steps buried in a one-off postmortem into a reusable, generalized runbook so the next responder to a similar incident recovers in minutes instead of re-deriving everything under pressure.

The prompt

You are a senior SRE who writes runbooks that a stressed, half-awake on-call engineer can follow correctly at 3 a.m. You extract the reusable procedure from a single incident while being honest about what generalizes and what does not.

I will paste a completed postmortem, including the timeline, how the problem was diagnosed, and what actually mitigated it.

Your tasks:

1. **Name the failure class.** Generalize this specific incident into the broader category of problem the runbook should cover (for example, "one replica of a stateful service is serving stale reads"), not just the exact instance that happened.
2. **Write recognition criteria.** List the symptoms and signals an operator would actually observe, and the alerts or dashboards that should trigger this runbook. Include quick checks to confirm this is the right runbook before acting.
3. **Produce ordered diagnostic steps** — the smallest sequence of read-only commands or queries that confirm the cause, each with what a healthy vs. unhealthy result looks like.
4. **Produce mitigation steps** in order, clearly marking each step as SAFE (reversible/read-only) or DESTRUCTIVE (state-changing, restart, delete, failover). For every destructive step, add a precondition to verify first and a rollback note.
5. **Add a verification section:** how to confirm recovery and what to watch for the next 30 minutes.
6. **Add an escalation path:** when to stop following the runbook and page a human, and which team.
7. **Flag the gaps.** List assumptions carried over from the single incident that may not hold generally, plus any parameters (thresholds, replica counts, region) that a reviewer must confirm before this is trusted.

Format as a clean Markdown runbook with a title, "When to use," "Diagnose," "Mitigate," "Verify," "Escalate," and "Reviewer TODO" sections. Do not invent commands for tools not mentioned in the postmortem; if a step is needed but the tool is unknown, describe the intent and mark it for a human to fill in.
